TREATMENT OVERVIEW
A Transforaminal Epidural Steroid Injection (TFESI) is a highly precise and targeted procedure designed to relieve pain that radiates from the spine into an arm or leg. Unlike other epidural techniques, this approach delivers a concentrated dose of anti-inflammatory medication directly to a specific nerve root, making it exceptionally effective for conditions like sciatica and cervical radiculopathy.
PROCEDURE DETAILS
For a TFESI procedure, you will be positioned lying on your stomach to provide optimal access to the spine. After sterilizing the skin, a local anesthetic is administered to numb the area, ensuring your comfort throughout the process.
The key to this technique is the path of the needle. Using fluoroscopy (live X-ray), your physician guides a thin needle from the side of your spine into a specific opening called the neuroforamen. This is the "tunnel" through which a nerve root exits the spinal canal. This approach allows for unparalleled precision.
Once the needle tip is confirmed to be in the correct position next to the inflamed nerve, a contrast dye is injected to ensure the medication will spread effectively. Finally, the therapeutic mixture of a corticosteroid and local anesthetic is delivered directly onto the source of irritation, maximizing the medication's effect.
TREATMENT ADVANTAGES
The transforaminal approach offers distinct advantages, making it a preferred option for treating specific types of radiating nerve pain with exceptional accuracy and effectiveness.
This technique delivers medication directly onto a single, specific nerve root, providing a more concentrated and potent anti-inflammatory effect where it's needed most.
By targeting a single nerve, a successful TFESI can definitively confirm which nerve is causing the pain, providing crucial information for future treatment or surgery.
It is particularly effective for radicular pain (sciatica, arm pain) caused by a herniated disc or spinal stenosis at a specific level of the spine.
The powerful corticosteroid directly targets the inflammation and swelling around the nerve root that is causing the compression and pain.
This outpatient procedure offers a safe and effective non-surgical alternative, helping patients avoid or postpone the need for major spinal surgery.
A TFESI is performed in-office and typically takes less than 30 minutes, allowing you to return home the same day with minimal downtime.
IMPORTANT INFORMATION
TFESI is a very safe procedure when performed by a skilled physician with fluoroscopic guidance. While complications are rare, it is important to be aware of the potential risks, most of which are mild and temporary.
Important: Your safety is our top priority. Please provide a complete medical history, including all medications and allergies, to help us minimize any potential risks.
PREPARATION GUIDE
Proper preparation is key to a successful and smooth procedure. Please review the following important steps to ensure you are ready for your appointment.
Ensure you have a thorough review with Dr. Fura. Disclose all allergies, especially to medications like contrast dye.
Stop blood thinners and other specific medications as instructed by Dr. Fura before your procedure.
Do not eat or drink after midnight. You must arrange for a responsible adult to drive you home afterward.
Wear comfortable, loose clothing. Avoid jewelry and leave valuables at home for safekeeping.
RECOVERY GUIDE
Your recovery after a TFESI is typically straightforward. Following these simple guidelines will help ensure a smooth recovery and the best possible outcome.
Plan to rest and take it easy for the remainder of the day. Avoid driving and strenuous activities. You can use an ice pack on the injection site for comfort.
As the local anesthetic wears off, it's normal for your original pain to return temporarily. You can gradually resume your normal, light activities as tolerated.
The steroid medication will begin to take full effect, reducing inflammation and providing significant pain relief. This is often when patients experience the most improvement.
Attending your follow-up appointment is essential to discuss the results of the injection and to create a comprehensive plan for your continued care.
Please don't hesitate to reach out and request an appointment. We look forward to meeting with you, addressing your concerns, and working together to enhance your quality of life.
